How it works

Getting started

1

Make a referral

Families can self-refer or be referred by schools, hospitals, GP's, or FACS. Call us to find out what's right for your situation.

2

Initial assessment

A Uniting worker meets with your family to understand your situation and what support would help most.

3

Support plan

Together we develop a plan — practical goals, who does what, and how we'll measure progress.

4

Regular review

We check in regularly and adjust the plan as circumstances change. Support can be short-term or longer term.
